She came along really looking for<escape V=".br" />alternatives to cholecystectomy as "there is no way she is having that".<escape V=".br" /><escape V=".br" />As I explained to her, her dilated bile duct really suggests that her gall<escape V=".br" />bladder is not working and it has not been working for many years. Even if<escape V=".br" />we could clear her gall bladder with shock wave therapy or dissolving<escape V=".br" />agents the odds are that it still would not work and form stones again<escape V=".br" />rapidly. It also provides a reservoir for new stones to enter the bile<escape V=".br" />duct and a place where biliary mud can form and later solidify in the bile<escape V=".br" />duct to form soft primary duct stones and then you end up in a cycle of<escape V=".br" />chronic infection which in many cases can never be satisfactorily resolved. <escape V=".br" />Several patients have regular clean outs of their bile duct because they<escape V=".br" />continue to form soft primary duct stones.<escape V=".br" /><escape V=".br" />I think I finally got through to her that cholecystectomy is the best<escape V=".br" />option and the option most likely to be beneficial to her and that<escape V=".br" />following any other path is probably higher risk although without a crystal<escape V=".br" />ball it is always hard to predict.<escape V=".br" /><escape V=".br" />I suggested she discuss it with you and make an appointment to see John<escape V=".br" />Andrews with a view to cholecystectomy and hopefully this will happen<escape V=".br" />without further ado.<escape V=".br" /><escape V=".br" />Thank you for asking me to review her.<escape V=".br" /><escape V=".br" /><escape V=".br" />Yours sincerely,<escape V=".br" /><escape V=".br" /><escape V=".br" />Thomas Anderson<escape V=".br" />Gastroenterologist.<escape V=".br" /></OBX.5><OBX.11>F</OBX.11></OBX></ORU_R01.OBXNTE><ORU_R01.OBXNTE><OBX><OBX.1>2</OBX.1><OBX.2>FT</OBX.2><OBX.3><CE.1>PIT</CE.1><CE.3>AUSPDI</CE.3></OBX.3><OBX.5>001 Medical-Objects 07 01/07/1996<escape V=".br" />002 <escape V=".br" />003 Report Run Number: 1 Created: 17/04/2005 at 22:51:04<escape V=".br" />004 Surgery: XXX Reports: 24/02/2005 00:00:00 to 24/02/2005 00:00:00 <escape V=".br" />009 ---------------------------------------------------------------------------<escape V=".br" />010 WHITE,DR.
